HISTOPATHOLOGICAL PROFILE AND P53 EXPRESSION IN VERRUCOUS CARCINOMA CASES AT A TERTIARY CENTRE

Abstract

Verrucous carcinoma is a low grade, well differentiated variant of squamous cell carcinoma and is a very rare entity. The oral cavity is the most common site of this tumor. Other sites being larynx, pyriform sinus, esophagus, nasal cavity and paranasal sinuses, perineum and the leg etc. To study the histopathological profile and expression of p53 in cases of verrucous carcinoma. 60 cases of verrucous carcinoma were collected. The H & E stained slides of verrucous carcinoma were reviewed to ensure conformity with accepted diagnostic criteria. The paraffin embedded blocks of the above cases were collected. Immunohistochemical studies were done on all the above casesby using Primary antibody, p53 as per standard IHC protocol. A dark brown precipitate within the nucleus confirmed the presence of p53.The stained slides were examined by light microscopy and graded from 0 to 3. Male to female ratio is 3:2. The age ranged from 30 - 89 years. Majority of cases were in between 60-69 years age (26.66%). 40 cases were from oral lesions (66.66%), and 20 from other sites (33.33%). History of tobacco intake was present in 35 cases of the 40 patients (87.5% cases) with oral lesions. Number of oral lesions were significantly more than those at other sites (p < 0.05). The microscopic feature of the cases conformed with the accepted diagnostic criteria.p53expression was detected in 53 out of 60 verrucous carcinoma (88.33% cases). Its location was mostly confined to the basal or suprabasal layers p53 was positive in 92.5% of oral lesions and in 80% of lesions at other sites. Majority of oral verrucous carcinoma as well as verrucous carcinoma of other sites were of grade 2, followed by grade 1. There were no grade 3 lesions. 60 cases of verrucous carcinoma were found in a 5 year study period. Histopathology conformed to the accepted diagnostic criteria. p53 protein was present in 88.33% of cases, and its expression was accentuated in the basal cells and suprabasal cells. The expression of p53 protein in the basal part may indicate basal part of VC have relatively higher p53 mutation than non-basal part of VC.
