EVALUATION OF SINONASAL DISEASE BY CT SCAN AND ITS OPERATIVE CORRELATION IN PATIENTS IN TERTIARY CARE HOSPITAL (SILCHAR MEDICAL COLLEGE & HOSPITAL)

Abstract

Paranasal sinuses computed tomogram (CT) scan are important for functional endoscopic sinus surgery (FESS) as their information assist the surgeon in pre-operative planning. The diagnostic algorithm for sinus diseases continues to evolve along with the advances in imaging modalities. Plain radiographs were once the mainstay of diagnosis of sinus disease and now have been replaced by high-resolution CT for the evaluation of sinus inflammatory diseases. This study aimed to show importance of CT scan in evaluation of anatomical variations to prove a correlation with disease process and extent of disease in sinonasal polyposis patients. The period of the study is from March 2017 to April 2018.All chronic sinonasal disease patients, after thorough history, general examination and thorough ENT examination, were examined by nasal endoscopy and sinus CT scans. In this hospital based prospective study 55 patients with symptomatic inflammatory sinonasal diseases were evaluated. CT diagnosis is correlated with final diagnosis obtained from findings of nasal endoscopy/Functional Endoscopic Sinus Surgery. Statistical analysis was done by descriptive and inferential statistics using Test statistics (sensitivity, specificity) Age range of the patients was 12-70 years (mean 34.58) of which male cases were more than female (1.39:1).Most of the patients came with complains of nasal obstruction (63.63%). 51 cases (92.7%) of the cases had anatomical variation of which Agger nasi cell (72.72% i.e., 40 cases) was found in most cases. On correlating CT diagnosis with final diagnosis, 50 cases (90.9%) chronic Sinusitis had strong agreement with intraoperative findings, while 5 cases (9.1%) had week agreement, P value in all instances was <0.05, i.e. <0.0001, indicating the significance of the findings. CT is the diagnostic modality of choice in evaluation of various inflammatory pathologies and associated complications thereby planning the further management of the patient.
