Abstract

One of the commonest chronic granulomatous infection in the developing world is Tuberculosis. Tuberculosis in the otorhinolaryngeal region is one of the rarer forms of extrapulmonary tuberculosis. Amongst the extrapulmonary tuberculosis most common is tubercular lymphadenitis. The present study has been conducted to assess the different clinical manifestations of tuberculosis in head and neck region. A total of 80 cases in the age group of 15 to 50 years who came to the Department of ENT, Jorhat Medical College with tubercular manifestations of otorhinolaryngeal region were studied. The duration of study was two years between Febuary 2015 to January 2017and included both out patients and in patients. After thorough clinical history was taken and physical examination was done,patients were advised routine blood tests including chest X-ray, Montaux test and sputum for acid fast bacilli. Diagnosis was based on Fine needle aspiration cytology and Histopthological examination. Out of all cases of tubercular manifestations of otorhinolaryngeal region- majority of the cases had cervical lymphadenopathy (80%). There were four cases each of tuberculosis of larynx and submandibular gland. Other sites with tubercular presentation were tongue with cheek abscess, tubercular retropharyngeal abscess, tubercular sinus tract infection of neck and pre auricular sinus tuberculosis. All the cases were proven histopathologically and all the patients had shown improvement with anti-tubercular therapy. Although less common in otorhinolaryngeal region, a high index of suspicion is necessary as the tubercular manifestations may have similar clinical presentation with head and neck malignancies and other chronic infective and non-infective conditions.
