Abstract
EVALUATION OF MEDICATION ERRORS IN ANAESTHETIC DEPARTMENT IN SPECIALIZED CONSULTATIVE CARE HOSPITAL

Introduction: Medication errors are defined as errors that are made at any step of drug administration, prescription, or dispensary by the health care professionals which may lead to problematic clinical conditions in patients within or out of hospital settings. Thus, from prescription to administration of drug, at any step medication errors can take place. It was reported in a study conducted in India that, nearly 5.2 million cases are reported yearly in India only due to medication errors and medication related adverse effects. Aims and Objectives: To evaluate the pattern of medication use in Anaesthetic department. To assess the types and frequency of medication errors in hospitalized patients of Anaesthetic department. Materials and Methods: Study was carried in Anaesthetic department of a 350 bedded hospital in India by name RVS Multi Specialty Hospitals. The perfect plan of the study in the form of protocol was submitted to the Institutional Ethics Committee for getting approval for carrying out the study in the hospital. All the health care professionals are prior explained about the study with the aims, objectives and the process involved in the study. Results and Discussion: It was observed in the study that prescription errors accounted with 1118 patients among 400 patients of overall study population with a statistical percentage of 69.41% of overall drug errors reported which included incorrect drug selection, doses not written leading to written errors, prescriptions not mentioned with frequency of drug use, and written wrong brand name by the physician. Other type of drug errors is administration errors which was reported to be in 52 patients with a percentage of 30.58% of overall drug errors that have occurred in 170 patients of 400 study population. Conclusion: It can be reported from the study that the frequency of prescription errors are more than 50% of the drug errors are occurred by prescription errors, so, better focus on prescription of medication by the doctors and better update on the hospital’s formulary and the brands of various drugs available in the hospital makes it helpful for the physicians for minimizing the prescription based errors.
